Aesthetic surgery aims to improve the appearance of the person. Aesthetic nose surgery or “RHINOPLASTY” is the reshaping of the nose to improve the appearance. Reconstructive rhinoplasty is performed to correct the deformity caused by congenital defect or injury. Reconstructive surgery may also be required for noses that have been severely damaged after surgery. Rhinoplasty can also be performed in conjunction with surgery to relieve nasal congestion caused by internal deformity.

SUITABLE CANDIDATES FOR RHINOPLASTY

The earliest recommended age for rhinoplasty is 16-17 years, when 90% of nose growth is completed. We find the age of 17 for girls and 18 for boys appropriate. In older people, the variable that influences the decision is health rather than age.

You may consider rhinoplasty if you have any of the following conditions:

  • If your nose seems too big for your face,
  • If there is a bump or depression in the profile image of your nose,
  • If the front view of your nose is too wide,
  • If your nose tip is drooping,
  • If your nostrils are wide,
  • If you have a crooked or crooked nose,
  • If you have a blockage in your nose that prevents you from breathing.

The first step that a person considering rhinoplasty should take is to consult a plastic surgeon. Keeping in mind that the desired result is correction, not perfection, one should openly discuss their expectations about looking and feeling better after surgery. One of the most important factors to be determined before any plastic surgery attempt is emotional balance. Rhinoplasty can give a new shape to your nose, but it cannot direct the flow of your life. Plastic surgery improves your appearance and refreshes your self-confidence, but the rest is up to you. After a physical examination of the nose, the surgeon will discuss with the person other variables that affect the decision about the method. During the first interview, the surgeon will explain specific details about your situation, such as the surgical technique to be used, anesthesia, where the surgery will be performed, and what this operation can realistically accomplish.

Thousands of rhinoplasty are successfully performed every year. You should still be informed about the possible dangers of surgery and the special risks associated with rhinoplasty. Post-operative complications (such as infection, etc.) are rare when first applied to correct the appearance. Poor recovery may require second surgery. The risk of complications is reduced by carefully following the surgeons recommendations during the recovery period. Rhinoplasty fees and hospital prices vary widely.

In your first meeting with your plastic surgeon, you should be careful to get complete information about the following:

  • The medical treatments you will receive,
  • Allergies that will create conditions that can cause congestion or breathing difficulties,
  • How often will you use nasal sprays that help relieve congestion?

As a result of your interview, if you decide to have aesthetic nose surgery and your surgery day is determined, your surgeon will inform you about the procedures you need to do just before and after the surgery.

You should avoid certain medical treatments that may complicate your surgery or recovery process.

You should stop all aspirin and alternative medicine drugs 10 days before the surgery.

You must stop smoking for a certain period before and after the operation.

You will be informed about the arrangements to be made for help and care after your surgery.

SURGICAL METHOD

The technique to be used for your surgery depends not only on your surgeons preference, but also on what you want and expectations from the outcome of the surgery. The surgeon should design the nose shape that he finds suitable for you, present it to you on the computer, and evaluate whether it is in harmony with your expectations. Aesthetic rhinoplasty is applied to reduce or enlarge the size of the nose, reshape the tip, correct the bump of the nose, and proportion the angle between the upper lip and the nose. In some cases, it is necessary to add texture to the face (forehead, cheekbones, chin, etc.) in order to correct the contour. One or all of these changes are performed in a single operation. The scope of the method depends on what changes are desired and what the surgeon deems appropriate. The surgery can be performed in an outpatient center or hospital in line with your surgeons recommendations.

In most nose surgeries, the incisions are made inside the nose wings and worked from there. This is called “closed rhinoplasty”. In other cases, the surgeon performs open rhinoplasty by making a small incision in the strip that bisects the nostrils opposite the columella.

The incision provides a gateway for the surgeon to cut, shave, reshape, and maneuver through bone and cartilage to alter its appearance. The nasal hump is shaved using a chisel or saw-like instrument, and then the nasal bones on the two sides are cut and brought closer together with a chisel to form a narrower bridge.

Cartilage incisions make the tip of the nose smaller and provide a better contour. In order to correct the angle between the nasal root and the upper lip, the tip of the nose is lifted as necessary. This process is performed by shaving the cartilage in the middle of the nose through the incision made on the wings. If a large bump is corrected from the ridge of the nose, the base of the nose will be disproportionately wide. In some patients, the nasal base is structurally wide. To narrow it, small skin triangles are removed from the base and the nasal wings are brought closer to the midline. After the cartilage and mucous membranes are closed by suturing, the nose is bandaged. Following this, a plaster splint is applied to the nose to protect the bone and cartilage in the new position.

Often, nasal packings can be placed to protect the middle cartilage of the nose, especially if surgery has been performed on the middle cartilage (deviation).

(The use of tampons has been abandoned by us in recent years)

The method usually takes between one and a half to two hours, but may take longer depending on the extent of the surgery.

POST-SURGERY HEALING

After the surgery, there may be a slight pain that is easily relieved with medications. You will be asked to lie with your head slightly elevated and apply cold compresses to your eyes to reduce bruising and swelling. In very rare cases, nasal packings, if placed, are removed one day later. Nasal plaster is usually removed in the first week of surgery and a bandage is applied for a week to prevent swelling in the nose. Your surgeon may recommend using the nasal splint at night. The bruises around the eyes begin to fade within a few days after surgery and usually disappear in the second week. Slight swelling in the nose gradually regresses, some lasting for months.

Since most aesthetic rhinoplasty is done through the nose, there are no visible scars. Only if the method requires narrowing the wide nasal wings, an incision is made in both nasal wing folds that will leave a barely noticeable scar.

It is unrealistic to expect a sudden transformation after surgery. Normal recovery is a slow process that requires time for the definitive result to be noticed. It may take weeks, months or even a year in some cases for the nose to take its final appearance.

Although you will be on your feet in a day or two after the surgery, your plastic surgeon will recommend a suitable program for you to resume your normal routine life. Activities that increase blood pressure, such as running, swimming, and even bending forward, should be restricted for the first few weeks to allow for proper recovery. The decision to return to work depends on the degree of swelling and discomfort of the person, 15 days is a suitable time.

If you have a bruise, the use of cosmetic materials is allowed to cover it up. However, surgeons recommend that their cases avoid the use of cosmetic materials that will require rubbing hard for several weeks after surgery. Sunscreen creams are recommended until the bruises subside. While everyones recovery rate differs, you might think that your recovery will generally follow the following process.

First day;

During this early stage of recovery, you should rest with your head slightly elevated to reduce swelling.

You should minimize your movements.

You may experience bleeding and congestion in the form of a small amount of leakage. This is normal.

In Ten Days;

Your swelling peaks and starts to go down.

Your bruises begin to decrease.

You can easily apply make-up to cover your paleness.

If any, your stitches will be removed.

You can go back to your school or work that is not very busy.

A few weeks later;

You can resume most of your normal activities and exercises. (4th week)

Most of your swelling will go down.

Your numbness will subside, but it may take several months to fully return to normal.

If there is no bleeding in the white of the eye, you can use contact lenses from the third day.

You can start using glasses. You can put it on the splint we gave you. (week 6)

(In the early stage of recovery, patients often keep their glasses with tape to keep their noses comfortable.)

In the months following your surgery, you must protect your nose from sunlight. It is important to use sunscreen creams to protect your skin.

RESULTS AFTER RHINOPLASTY:

The degree of improvement depends on the extent of the corrective effort and the basic structure of your nose, cartilage and skin. A striking result is achieved in a clearly deformed nose, while a slightly large nose can also be successfully corrected, but the result may not be obvious.

Its common for relatives and friends to say they dont see much of a difference after some types of rhinoplasty. Do not take such reactions as a sign of failure, on the contrary, if it is a "better" and "natural" nose, it will not go unnoticed. Ultimately, the goal is to create a nose that is skillfully blended in proportion to the general features of the face, enhancing the beauty of your face, rather than creating a striking new nose. Sometimes minor corrections and revisions may be required and may require additional surgery.

The factors that affect the results positively or negatively in patients who have undergone aesthetic nose surgery by applying a valid scientific technique are listed below:

  • Factors that positively affect the results:
  • Factors that negatively affect the results:
  • Factors that negatively affect the results:
    • Having had previous surgery

    • Having had multiple surgeries before

    • The need for rib and ear cartilage grafting with a major intervention from the beginning in the surgery

    • Unhealthy skin with surgery

    • The operative skin is hard, scarred, and damaged in a way that makes treatment difficult.

    • It is necessary to intervene in these patients as a kind of savior. In these cases, results such as first aid can never be achieved, but their very bad appearance allows the person to be adjusted to an acceptable extent so that they can go out in public comfortably. Multiple correction surgeries may be needed in this group.
